Tokyo: all 23 ward offices, each address as published by the city itself. Which one is yours is decided by your address, not by choice.

Within 14 days of arriving — register your address
This is the first errand of your life in Japan and everything else waits on it: insurance, pension, a bank account, a phone contract. Bring your residence card; they write your address on the back.
What to say at the counter
転入届を出したいです。 Tennyū todoke o dashitai desu. I would like to file a move-in notification. -
National Health Insurance — same visit if you can
If no employer enrols you, you join here, and cover starts from the day you registered — not the day you walked in. Delay it and you are billed for the gap anyway.
What to say at the counter
国民健康保険に加入したいです。 Kokumin kenkō hoken ni kanyū shitai desu. I would like to join national health insurance. -
National Pension when you have no company
Students and the self-employed enrol here rather than at the pension office. Ask about the student exception or an exemption in the same conversation if money is tight.

国民年金の手続きもここでできますか。 Kokumin nenkin no tetsuzuki mo koko de dekimasu ka. Can I also do the National Pension paperwork here? 学生納付特例を申請したいです。 Gakusei nōfu tokurei o shinsei shitai desu. I would like to apply for the student pension deferral. -
Registering your seal (印鑑登録)
A registered seal plus its certificate is what a car purchase or some rental contracts demand. Bring the seal itself and ID; the certificate is issued afterwards, any time you need one.

印鑑登録をしたいです。 Inkan tōroku o shitai desu. I want to register my personal seal. 印鑑証明書をください。 Inkan shōmeisho o kudasai. A seal-registration certificate, please. -
Certificates you will be asked for again and again
Residence record, taxation and tax-payment certificates: needed for visa renewal, loans and school applications. Many can be picked up at a convenience store once you have a My Number card.

住民票が一通ほしいです。 Jūminhyō ga ittsū hoshii desu. I need one copy of my residence record. 課税証明書と納税証明書がほしいです。 Kazei shōmeisho to nōzei shōmeisho ga hoshii desu. I need the taxation and tax-payment certificates. コンビニでも取れますか。 Konbini demo toremasu ka. Can I also get it at a convenience store? -
The My Number card
Not compulsory, but it turns most of the above into a convenience-store errand and doubles as photo ID. Applications are taken here; the card arrives weeks later.

マイナンバーカードを申請したいです。 Mai nambā kādo o shinsei shitai desu. I would like to apply for a My Number card. -
Family matters — birth, allowance, nursery
Birth notification within 14 days, child allowance from the month after you apply (it is not backdated), and nursery applications that run on a strict annual calendar. Ask about all three at once.

子どもが生まれました。出生届を出したいです。 Kodomo ga umare mashita. Shusshō todoke o dashitai desu. My child was born — I want to file the birth notification. 児童手当の申請をしたいです。 Jidō teate no shinsei o shitai desu. I want to apply for the child allowance. 保育園の申し込みについて聞きたいです。 Hoikuen no mōshikomi ni tsuite kikitai desu. I want to ask about applying to a nursery. -
Moving again — within the ward or out of the city
Moving inside the same ward is one form; leaving the city means a moving-out notification here and a moving-in notification there within 14 days. The gap is what causes insurance trouble.

同じ区の中で引っ越しました。転居届を出したいです。 Onaji ku no naka de hikkoshi mashita. Tenkyo todoke o dashitai desu. I moved within the same ward — I want to file a change-of-address notification. 他の市に引っ越します。転出届をお願いします。 Hoka no shi ni hikkoshimasu. Tenshutsu todoke o onegai shimasu. I am moving to another city — I need a moving-out notification. -
When the Japanese is too hard — the multilingual counter
Most city halls have interpretation, by phone or in person, and free foreign-resident consultation. It is a service the ward pays for; use it rather than guessing at a form.
What to say at the counter
外国人相談窓口はありますか。 Gaikokujin sōdan madoguchi wa arimasu ka. Is there a consultation desk for foreigners? 通訳サービスはありますか。 Tsūyaku sābisu wa arimasu ka. Is there an interpretation service? 日本語があまり話せません。ゆっくりお願いします。 Nihongo ga amari hanasemasen. Yukkuri onegai shimasu. I do not speak much Japanese. Slowly, please. この書類の書き方がわかりません。 Kono shorui no kakikata ga wakarimasen. I do not know how to fill in this form. 窓口は何番ですか。 Madoguchi wa nan-ban desu ka. Which counter number should I go to?
The half nobody prepares for. These come at you in Japanese — recognise them and the visit is easy.
